How to document: date, exact symptom or event, severity, duration, outcome, any medical attention sought. Detailed documentation is far more useful than a vague report.

bench_notes, post #49: This follows post #46 rather than contradicting it. Psychological adverse events: mood changes, anxiety, or suicidal ideation warrant discussion with a mental health professional. These are rare but real. Go to post

Where to report: in many countries there is a pharmacovigilance system where adverse events can be reported. FDA MedWatch in the US, Yellow Card in UK, EudraVigilance in EU.

Coming back to post #121, because the follow-up matters more than the original answer.

Pancreatitis is a genuine serious adverse event to know: severe epigastric pain, back pain, elevated lipase (≥3× upper limit of normal). If this constellation appears, stopping the drug and seeking urgent evaluation is appropriate.

Acute kidney injury: rare but reported. Symptoms (decreased urine output, creatinine elevation) warrant urgent evaluation. Severe dehydration during weight loss is a risk factor.
